Should I remove all of the old amalgam before placing the OVC?

Yes, unless there is amalgam close to the nerve you would generally always remove it before the OVC procedure. In cases where if the amalgam has left the dentine very dark this can show through the tooth when it is completed – in these cases you can use an opaquer to help mask the dark colour.
